2004 A worker in New Burn North Carolina dies while dealing with an upset condition at the sawmill stacker. 2004 Broken piece of wood is jammed between the carriage and the frame of the stacker.
Upon trying to free up the carriage, slack is created in the carriage chain as the piece of broken wood becomes jammed in one of the tracks. Upon pounding out the broken piece employee fails to notice the slack built up in the chains and is crushed to death as the carriage drops pinning him between the frame and the outfeed roll case.
2009 Worker Crushed to Death in Slave Lake Alberta. Employee crushed and killed while stacking Veneer at Alberta Plywood Ltd. Spring loaded veneer stacking unit descended onto the employee crushing him.
Both of these cases involved an apparent lack of hazard recognition regarding stored energy. As Western Forest Products has many devices which contain similar hazards (stackers, sorters, press rolls, etc) please take time to identify and discuss these hazards with people at your site.
