An experienced worker was fatally injured when the Madill 3800 log loader he was off-loading from a marine barge slid off the loading ramp and tipped over on its side; trapping the worker in approximately 8 feet of water (The tide was higher than shown in the photo).

  

Learnings and Suggestions: 

Operations involved in loading and unloading equipment onto barges should:

  • Review their current procedures to assess whether they have fully addressed the hazards involved including their procedures for reviewing changing conditions such as tide and weather.
  • Review their emergency response plan in the event a piece of equipment enters the water.
